Haven't tried one myself (yet), but I've heard that the modern seal
works very well (once you get it to seal), even in engines with a fair
amount of blow-by.  Ken G. at BFE has written a series of articles on
how to make them seal, though, so it is apparently not entirely
straightforward.  (ISTR him writing about an unnamed Morgen mechanic who
tore his engine down 6 different times, trying to fix all of the leaks. 
He did finally succeed, but what a PITA.)

The crank has to be machined (and ISTR Ken saying that the proper
finished diameter was one of the critical items), and there may be a
little cleanup work required on the block (the edge that the seal
housing covers has to be radiused a little, so the housing sits flat),
but otherwise it's a bolt-on.

I believe Ken will sell the conversion a little cheaper than $130 ...
