Ken,

Question 1, everyone else has answered already, far better than I could

Question 2, I would say they are rare enough that they are probably
worth restoring from almost any state of disrepair - I don't think
you'll be able to get (identical) replacements for them.  Of course,
only restore them if you actually like them.  I've only ever seen them
in pictures, but I have to say I can't see how anyone can not like them
;-)

Question 3, check out the yellow pages.  There's a couple of "wheel
shops" near me - mostly they specialise in alloys, but if somewhere like
this can't help, I'm sure they could point you in the right direction.
