Hmmm, maybe someone could make up a "standard" label format and create a
  template file that would fit it on some standard sized label. It could 
  be posted on the website (probably as a .doc file) and people 
  could download it, type in their info, pick up a package of the 
  correct sized labels and print them out.

I'm thinking this would be easier for both the people making the labels
  (no need to figure out what info is needed), and Pat Kelly (she won't
  have to deal with different people putting the info in different
  arraingements).

I'll volunteer to create the template if I can get a good listing of
  what needs to go on it. It would be ideal if someone could scan a
  card and email me the image so I can make the label as similar to the
  card as possible.
