I wasn't sure either, but thought perhaps John was referring
to the flexible joint on the steering column?  If so, the
problem may be that shoulder of the bolt has to pass through
a groove on both sides - e.g. the column and the little stub
that comes out of the rack have these grooves.  The joint
needs to be moved up and down until these grooves are lined up
with the bolt pass through hole on the flexible joint.

The flexible joint can also be prised open slightly if it's
a really tight fit - when you tighten down the nut, it should
close up tight again.
