78 Clutch replacement

I finally got a chance to work on my grounded spit.  The problem was the
clutch.  The clutch plate broke, there is now a outter ring and an inner
hub.  A picture of it is at the following link:

There was only around 15K miles on this clutch and it was made by B&K.  Anyone
ever seen such a failure and kown what can cause this?  The only thing I can
think of is the output shaft was binding with the plate and cause it to flex
too much and fail.  I guess I was lucky it finally gave up in my parents 
drive way.

I'm not that happy with the new clutch kit that I got.  Althought the
supplier advertised it was B&K the parts doesn't look to be in best of quality.
The clutch plate is solid without grooves and it's thinner and can only 
have 1.5mm of wear before hitting the rivets. But they are stamped with
markings saying they were made in England.
