Lock washers, like Nyloc nuts - are meant to be used once only.

It's the dulling of the 'cutting' edges after repeated doing-up and un-doing
that render a lock washer useless.

A new lock washer will hold!

                Use a drop of LocTite on the threads and torque it properly.
It should stay.
