Lock washers, like Nyloc nuts - are meant to be used once only.
It's the dulling of the 'cutting' edges after repeated doing-up and un-doing
that render a lock washer useless.
A new lock washer will hold!

Not sure whether it has a lock washer, but lock washers are
really useless,
I understand. According to one quality fastener
manufacturer rep, lock
washers are "not very effective, but you keep buying them
and we will keep
making them!"
