Use a drop of LocTite on the threads and torque it properly. It should stay.
Not sure whether it has a lock washer, but lock washers are really useless,
I understand. According to one quality fastener manufacturer rep, lock
washers are "not very effective, but you keep buying them and we will keep
making them!"
