Thanks to everyone who responded. I did a terrible job of describing the 
question as many suggested I gently file down the nut to expose the split pin 
hole. 

My problem was just the opposite... a bunch of bare threads exposed below the 
nut before you find the split pin hole. 

Bob Rich says he always puts a lock (split) washer on before nut and that 
would seem to solve the problem. Couldn't see a problem for adding a washer 
or spacer of some type but thought I might be missing some larger issue (like 
I assembled improperly). 

I'm working towards having all bits and pieces staying where they are 
supposed to be... (especially steering mechanisms) while whizzing down the 
road at speed!
