That's a caged needle bearing, and if your output shaft is rough then it'll just eat the needles. Assuming you want to avoid paying for a new output shaft, I'd just polish the end down and fit a bus

Yeah, I'd like to see a reputable 3rd-party analysis too. K&N make all sorts of "better performance" claims, and the warranty... but as far as I know they have *never* made public claims about better

The "carrier" (whatever it's called) into which the needle threads, has right-hand threads. You're working it from above, so turning the tool CW (to the right) draws the needle up into the air valve,
