Well, it's not really a "lockout" per se, it's really a switch that only closes
when the appropriate gear is selected.  Not completely foolproof, I'll grant
you, but not quite as bad as a switch that has to actuate to prevent engagement
in reverse.

I would also think that any reasonable driver would notice the problem, and not
force the car to back up when the OD has reverse locked up.  Feels like you're
trying to back over a curb or something.  And it's not like the OD just
instantly turns to mush, it will take a fair amount of force before any damage
occurs.

Gotta be a pretty strange leak, since the A-type valve actually bleeds oil away
from the clutch pistons.  A blocked relief vent can cause the OD to engage "by
itself", but usually only when the car is moving at a fair rate of speed (and
hence the OD pump is putting out oil).  Not likely to happen with the car
stopped.
