TR4 flat fan: parts flying off?

In the fall of 2003 I was driving south on I-75 about midway between 
Cincinnati, Ohio and Lexington, Kentucky, on my way to the SE Vintage Triumph 
Register meeting at Jekyll Island, Georgia, when the cooling fan on my '64 TR4
