Had the exact same problem with a rebuilt unit.  My problem was the 3/4
gear selector fork's prongs were  bent, ever so slightly  (They're bent
towards the front of the car, as the fork mechanism is jamed backwards
upon selecting 3rd, thus prohibiting full engagement of 3rd.).  Just
take the top cover off and inspect the fork again, perhaps coercing them
back a bit.

I don't think, but am not 100% certain, that the detent, balls and
springs are what keep the gears meshing and I agree with your synopsis
that it should happen in 4th as well, if these items did just that.
