Jim,

Last month I had a set of redline tires for my TR6 shipped from North
Carolina to Vancouver, B.C.

I had our company shipper look after the paperwork but the following
documentation was required to clear customs.

1. North American Free Trade Agreement - Certificate of Origin
2. Proforma Invoice prepared by Customs Broker ( GST payable is
determined)

As for a shipping company, I would recommend contacting local haulers
ie: Canadian Freightways and have them recommend company as well as call
local customs broker and have them advise you on the requirements for
importing used parts. The best way to crate or package is to have the
engine and trany put on  pallets and strapped and shrink wrapped.
