Pete, I can't now recall for sure how I got mine out although mine were all
there. I think I just used the shaft to drive them out. But then I had to
cut my shaft anyway to free the for as my pin was toast. The bushings are
not hard to remove as I recall.

The one thing I would recommend (I did this) is to use two per side --
especially on the lever arm side. There is room for this and the bushings
are relatively inexpensive.

Sorry I can't help you with the fork question -- mine had the boss and
that's where I put the extra bolt. My car is a real late 6 and the fork I
have in there is original to the best of my knowledge.
