Thanks for everybody's input.  I faxed BMIHT this morning with car and
credit card information.

The BMIHT website, http://www.ipl.co.uk/MG/herit00a.html still says no
credit cards, but the last update on the page is June 1995!

The reason the link to BMIHT did not work in Joe's post was the URL used
two letter "O"'s  instead of Zero's.  It happens to the best of us!

Hopefully I will see a certificate soon.
