A method I've had good luck with is to weld a nut to the bit of the
bolt or stud that sticks out from the surface. The nut gives you a
way to put a wrench on it, and the heat of the welding will destroy
any loctite or such like; it also causes the bolt to expand, which
can loosen a rust bond. If the stud is flush with the surface or
below it, it's possible to build it up enough (carefully! don't weld
it to the side!) that you can weld a nut on it.
