Erratic timing (timing light on the pulley showing a variation whilst the
engine is held at a constant speed) is one indication, although that can
also be caused by wear in the timing chain and gears, or the cam gear and
drive dog. My V8 had slightly erratic timing which became rock-steady after
replacing the cam and timing gears and chain, as part of other work.
Removing the distributor cap and seeing how much sideways movement there is
in the shaft is the best test.
