British Motor Heritage Registration

I am confident this question has been asked and answered here  before now, 
but.....
what is the proceedure to receive the certificate from the organization in 
England that provides these?  

Generally, I am led to believe the cost is reasonable (don't know how much), 
and I don't know who to write or fax, or if they take credit card, check or 
what information is required.
