Rocognition Forms

Hello all,

I would like to find a copy of the SCCA Recognition forms for a
Cortina  Mk1 and Mk2.
Unfortunately, the SCCA archivist was not able to find anything in his
collection.

Some of you fellow Cortinists (my spell check suggested that I spell
this Cartoonists) may have heard me ask this individually, but I thought
that the general listers may be able to point me in the right direction.
