But that little difference is huge.  Someone in the car with you is 
aware of your local environment, knows when you're negotiating a 
tough situation, knows when to stop talking, can even point out a 
potential danger, and understands and cooporates when you suspend the 
conversation for some traffic-related reason.  (If they just keep 
talking because they're too oblivious to notice, you tend to tune 
them out for a few moments.)  By contrast, someone at the other end 
of a phone connection may know that you are driving, but that's as 
far as it goes.  Similarly, when you're conversing with someone in 
your car your awareness is there; when you're talking on the phone at 
least part of your awareness is focused on that person.
