All of the things you have done so far sound appropriate, but I
would like to add one little thing for you to check.  I had a
similar experience in my Datsun Roadster, tried every cure known
to man (and alien), w/o luck.  Finally, in desperation and upon
the advice of a friend, we checked the muffler -- the baffles had
partially collapsed inside and allowed enough exhaust gas to pass
through at idle, but under load it "backed up" and choked off the
motor.

Granted, it is a long shot, but you may want to check the backflow
in any event.
