The 220v circuit is completed between the two hot wires. In effect, they
are 180 degrees out of phase with respect to each other, so if you meter
from either leg to neutral (or ground), you will see 110v, but across both
legs you will see 220v. Many 220v connections do include the neutral (eg
electric dryer outlet), to allow for connection of 110v devices like light
bulbs and fan motors.
