It doesn't take too much to make one either.  Friends with both the 
     eared and hex nuts have done this...
     
     Three eared:
     thick wall tubing notched to fit on one end, then welded to a piece of 
     steel plate appropriately shaped to hammer on.
     
     Hex:
     Thick wall hex stock or buy a big socket and weld to the lever arm.

There are two different tools, at least, on the market for avoiding 
the hammer. I have a fairly expensive one that includes a torque 
wrench fitting, from the Stainless Steel Brake company. There's
a less expensive one that is just a fold up arm with a welded 
bracket that fits over the knockoff ears, too.
