Best PC Driving Game/Wheel

What's the best (for a PC) game and driving wheel in terms of realism? 

Last year I got Test Drive 5 and a Thrustmaster NASCAR wheel/pedals 
connected to my soundblaster 16 card. This is way better than what I used to 
try years ago, but is there a better combo out there?

PS. I know the Playstation GT is supposed to be awesome, but I need 
something for a PC.
