This may be of help.  In the final throes of rebuilding the engine, the =
clutch plate had cracks where the spokes met the outer ring.  As the car =
has the Toyota Supra 5 speed from many years back, trying to match =
wasn=E2=80=99t happening.  My local shop had a source of Kentucky Clutch =
in Brooks, KY (kyclutch.com <http://kyclutch.com/>).  They rebuild or =
build new clutches for cars, trucks, race cars.  I know that have =
provided throw out bearings also.  They might be a good source.  I sent =
the broken clutch plate to them and a week later a new clutch plate =
arrived - the quality was far superior to the plate they replaced and =
the cost was $135.00.  Who knows, maybe they will have a solution or at =
least some good suggestions.
