Fan could be making intermittent contact somewhere with different sounds at
different speeds. If you do suspect a stuck valve that should be easy enough
to check by removing the valve cover, shake and observe.
I should think that if the timing chain tensioner went that it would jump time
and not run well at all, but there are greater minds than mine that can weigh
in on that possibility.
