Occurrs in 2nd, 3rd and 4th, only while accelerating or at a constant
RPM - does not occur on deceleration or when not under load.

Are you sure it is coming from the engine? I had a similar issue which
turned out to be the transmission. The 'under load' part seems to point
in that direction. Also since it started after the transmission
installation...

Is there a difference in different gears? Does it go away in fourth?
If so it is layshaft related. BTDT.
