In most cases, I was able to get a pair of Vise-Grips in to clamp the ears
of the cage onto the nut. Then lots of penetrating oil, some heat, another
pair of Vise-Grips, and lots of wiggling back and forth would walk the bolt
out of the nut.
The big problem on my car was not so much the threads inside the nut being
rusty, but the threads that protrude from the back of the nut. So each
cycle of turning the bolt out and then back in would clean a bit more rust
off of the protruding threads.
