Dave, I think you sent this to the wrong list. But to answer your question
I'd need to know more about your PCB layout software. Perhaps there is a pad
adjustment feature that needs to be turned off. Are you creating the fiducial
as a pad or as a copper fill area? How much more than .040 is you fiducial?
Is the mask pad the right size? Perhaps the paste mask generator is using the
solder mask layer and not the copper layer. Did you check the dashpot oil?
