One explanation might be that someone has put the wires into the cap
wrong, or even put them in right after the DPO deliberately put them in
wrong.

That's probably also the least invasive way to fix the problem.

The hole through the shaft and dog is not drilled on-center, the
original solid pin will not go fully into place (at least not without
excessive force) if you try to turn the dog 180.  However, it can be
done with a roll pin.
