What Jim added is important.  The personal information was a big clue.
When was the last time you handed someone your drivers license as you
told them off.  Ok, a couple of you have.

Spam wants extra traffic for the fun of stirring or jamming things up.
The person identified and address many times are NOT the Sender.   The
message headers need to be read carefully.  The person identified may be
a victim.  If anyone on this list let's their blood boil the actual
sender achieved their purpose.  If you repeat the message without care
you draw more people into the spammers purpose.

The service providers don't like this stuff anymore than any of us do.
The key for this type of issue is to find an authority with the ability
to cut off access or take legal action.
I would start with a question  to your service provider for help.  Even
if there is not enough information for them take definitive action, your
effort starts a record.  It may take complaints from multiple people to
find the cause.  Again the person that shows in the message is not
always the actual sender, don't help the spammer make that person a
victim.
