The metal sleeve is made of spring steel and is simply pressed on the shaft
and stays there by virtue of the triangular fingers which prevent the sleeve
from sliding back off by pressure against the square shaft.  If you can get a
small lever, screwdriver, etc. behind the sleeve you can usually work it off
by levering side to side. Alternatively, bend the triangular protrusion away
from the shaft, then rebend for installation. While the sleeve will not break
easily, be careful as they do not appear to be available new. The actual door
handle is held to the door by two small nuts inside the door which attach to
two threaded studs on the door handle..  If these nuts are really rusted they
usually just break off the  studs on the door handle as they do not seem to
be of very good quality metal. Entire new door handles are available.
