Assuming the fast idle lever is indexed correctly on the throttle spindle,
use the middle hole.
Using the top hole will decrease lift at the fast idle adjustment screw,
so decreases the maximum fast idle attainable. However, you will probably
need a longer adjustment screw.
The lower hole will increase lift, but the fast idle lever will probably
rest on the cam even with the adjusting screw removed, so you won't be
able to decrease the idle speed to normal.
However if the fast idle lever has been incorrectly indexed (pinned to the
throttle spindle in the wrong position), as long as it isn't too far off
the upper or lower hole on the cam can be used to give you back a
reasonable degree of fast idle adjustment.
