It does happen. Don Schroeder of Car and Driver was killed in a high-speed
test of a RennTech Benz about a year ago, and as I recall, two King of the
Hill Corvettes were toasted on a press junket in Europe several years ago.
To hear the automotive journalists tell it, cracking up a manufacturer's test
vehicle is not all that uncommon. Unfamiliar car, enthusiastic driver trying
to squeeze every ounce of performacne out of it, etc. etc.
Biased comment: Plus, those Audis are incredibly quick! Ask me how I know!
