My Dianosis would be, the because of the broken bearing guide thing. Your bearing was not held square. Because of that it did not spin (why should it?) , rather the fingures slid arround on the release bearing surface, wearing out the fingures etc. In conclusion, doen't matter how expensive your clutch kit is, if there is any play or wear in the releace bearing linkage your just throwing away your money.
