So it's quiet in 4th, whether the clutch is in or out,
and it's quiet when the car is at a full stop and your
foot is on the clutch. Is this right?

If it is, the problem is the needle bearing where the 
mainshaft rides inside the constant pinion shaft.

If you have a later style gearbox you might be able to
salvage it without ripping the whole box apart. Write 
back for more info.
