Any idea why it failed? Was the concrete cured enough? Were the instructions
poor? What brand was it?

I went through this a couple of years ago. As with all paint, prep
is the key. We were painting a new slab, did the acid etch, used
high quality epoxy recommended by several people ... and it didn't
fire properly. Peeled and lifted and was disgusting.
I ended up hiring someone to redo the whole job, with a warranty,
for about $3/sq foot including a clear urethane finish coat.
Don't want to scare you, but do want to warn you that these things
can go bad and you should be prepared for that eventuality...
