Sounds to me like throwout bearing or pressure plate. Motor out time. Sorry.
You'll know for sure when you split the motor from the tranny.
I would plan on replacing the throw out bearing, pressure plate, driven or
clutch plate and the spigot bearing.

And, from the tone of the list lately, I'd suggest you plan to purchase new
parts versus used parts.
