Sometimes, just sometimes, the gasket between the exhaust manifold and the 
exhaust system can fail in a way that results in a mechanical-type rattling 
noise.  Generally, however, the water pump is the prime suspect.  And even if 
you've tuned your fan belt to the perfect tension, these things have 
relatively short lives.
