My recommendation is to avoid lock washers altogether. Under severe
vibration they simply cannot be trusted.
Loctite is a far better solution; so are self- locking ("prevailing
torque") nuts. Of course, tightening the fastener to the right torque is
half the battle.

Anyone have the name of the company who manufactures the step  style, 
serrated or maybe "wedge" style 2 piece lock washers ?   Each of the
steps on each 
washer is suppose to face each other and prevent the  bolt from
lossening.  Hope
