Hmmm. This looks like a lot shorter version. It's been a long time since
I read this but the article I recall had numerous examples of attempts at
solving safety issues that caused accidents. I might be mixing and
matching articles.

But, there isn't much more there than a recital of how John Denver died
(fuel switch in a really, really bad location) and some moralizing on
bad
user interfaces.
The thing I've always wondered is, did JD know that the plane had been
"improved", and that (according to other articles I've read, not
mentioned
here) the "improvement" was why a previous owner had decided to sell the
plane?
