-Though I believe that a TR6 differential unit and driving flange is
identical to a TR4A-IRS they have different part #'s.
Looking at the remnants of the flange it appears that it broke/split
all at one time. Metal fatigue? Loose taper fitting? I suspect that
the best thing to do is getting a flange and axle and getting the
tapers to fit by lapping. Comments?
Should machining in another keyway be helpful? -i.e. have two keyways
or is this not needed.

The early parts fail in street use and were uprated by the factory. Use
the
later items; the key is wider. It fits with no mods.
Lapping the taper should help.
I check the nut torque in street cars, and most strongly recommend that you
do frequently!
