Tim,
Definitely the T/O bearing.
Rubber boot is cheap, and cannot be replaced with the trans in the car, so I
would replace it.
Pilot bushing. Again, cheap.
Speedometer drive gear. This can be replaced with the trans in the car, but
it is inexpensive and much easier to replace with the trans out.
