According to Cars & Parts magazine, you do this:

Use a hammer and punch to knock in the plug at the edge of its flat face,
up against the flange that holds it in. This should push in the metal some-
what, simultaneously pulling a part of the flange away from the side. Grip
the flange with locking needlenose Vise-Grips or similar and yank it out.

I have not tried this myself.  ;-)
