Make a template from cardboard or other sheet material you can easily cut.
Buy a piece of Lexan, glass shops carry it, and cut and drill it yourself.
The edges can be sanded much like wood. Lexan is easy to cut and cold-form
whereas Plexiglass is brittle and will shatter and crack. Downside is Lexan
will wear out quicker. Good luck!
