medical info that attaches to helmet or roll-cage?

Hey all, 

I decided to ask the experts (you all)  about something I saw in a magazine 
(either GRM or SCC).  It is a medical consent form and medical infomation.  
In the highly unlikely case (we all hope) that the driver is unconscious from 
a crash this information is attached to his/her helmet or roll-cage.  It is 
packed like a UPS packing slip.  I cannot find it in any of my magazines.  
Can anyone help me out?
