If you have a regular, not a space saver spare, you can either rotate it along
with the other 4 OR, never use it and then just 3 new tires when the old ones
wear out!
My wife normally gets 60,000 miles on a set of tires (4 tires, space saver
spare). We rotate, but only 15,000 miles or so.
