Phil---Your distributor may very well need a rebuild, especially if your
rotor is chewing up the inside of the cap towers, if/as it swings
eccentrically.
Some other things to consider...We know that our engines will idle
relatively smoothly whether the timing is set at 4 deg. ATDC or 12+
deg. BTDC, and anywhere in between. So if the timing is fluctuating,
this alone should not be the cause of the symptoms you describe.
Try setting the points gap at .010.
"Shorten" the length of the rotor's brass arm by filing off about .032
(to keep it from making physial contact with the tower terminals).
Do a compression or a cylinder leakdown test, noting if one or more is
seriously (20%) lower than the rest.

I have come to the conclusion that most of the lumpy running, missing
and stinky exhaust on my car is due to a worn-out distributor. The fact
that there is some play in the rotor/shaft and distributor caps last
about 1000 miles is further evidence.
I searched the archives and came up with a recommendation for Bob Sarama
as a rebuilder. Any others?
